When I accelerate in my car why does power transfer to the wheels?

When accelerating I sometimes get no extra power to the wheels. The engine revs go up, but other that that I get no response from the car. This is an intermittent fault, so it appears that something is beginning to fail. My car is a Ford Mondeo 1.8L (1998)
While driving the car at 30mph, put your foot down hard on the throttle. If your engine starts to rev higher and not in relation to your speed than you have a worn clutch plate.
